  • The program team does not have your address or personal information. They only have coded information, meaning your details are kept private with a special number instead of your name. The program team identified that your lab tests showed signs of kidney disease and shared this with Manitoba Health. Only Manitoba Health knows which number links to your unique number to your name and address.

  • You don’t have to make an appointment if you don’t want to. Participating in this program is completely up to you, and your decision won’t affect your current health care. You will automatically get a reminder letter in 3 months. If you decide not to participate, we strongly encourage you to talk with your primary care provider about your kidney health.

  • There are a number of ways that you protect your kidney function, like:

    o Staying actives

    o Eating a healthy diet

    o Not smoking

    o Keeping your blood pressure and blood sugar under control

    Visit My Kidneys My Health for more information on how to live well with CKD. If you already have a CKD diagnosis, talk to your doctor about medications that may help protect your kidneys.

  • To book your appointment, call the project office at 204-632-3541. Let them know you want to talk about the CKD Safety Net project. A team member will help set up your appointment at the kidney health clinic at Seven Oaks General Hospital.

  • Yes, your primary care provider will get a letter from the kidney health clinic about your visit. This is part of standard care for all visits to the kidney health clinic.
